All claims and supporting documents should be submitted via mail to the following address:

Supporting documents:

1.The original detachable receipt from the money order. (Purchaser's copy)
2.Money order for the processing fee specified on your claims form.
3.A photocopy of valid government issued identification.

REPLACEMENT PROCEDURES

(1) If the Money Order has not been cashed, the Purchaser may receive a refund of the value of the Money Order. To obtain a refund, the Purchaser must complete and sign the Claim Form on the back of the detachable Receipt stub or a full sheet Claim Form and send it, along with the non-refundable processing fee indicated on the Claim Form, to the address on the form. The Original Detachable Receipt-- not a copy -- must accompany the request. If it is not enclosed, your request will be delayed and may be denied. Allow at least 30 days for processing and mailing the refund.
(2) If you need to obtain a claim form you can:
(a) Go to www.moneyorderclaims.com you will need your money order number; OR.
(b) Return to the store and obtain a claims form from that location.
(3) If a Money Order has been cashed, a photocopy of the front and back of the cashed Money Order can be obtained by completing the Claim Form, and sending it along with a non-refundable $20 processing fee to the address on the Claim Form, or call 1-866-514-8801 and obtain a front and back copy of the cashed Money
